Asthma affects more than 15 million people in the United States one
third of those are children; It is the most chronic disease among children.
Although most people with asthma suffer mild to moderate symptoms, it can be
life threatening if undiagnosed or unmanaged. Although there is no cure for
asthma, there are reliable diagnostics and safe and effective asthma therapies.
A general goal of asthma management is to enable patients with asthma to live
full, active and productive lives. Symptoms need to be controlled so that they
are minimal or absent. To achieve this, the member must take a proactive role
in controlling the disease and preventing symptoms, rather than a reactive role
in relieving observed acute symptoms.
HCMTI has an Asthma Health and Lifestyle Management Program to
help you in tracking your individual asthma-related issues and assist you in
addressing your health concerns and medical care.
Enrollment in the program is strictly voluntary. There is no
cost to join the Asthma Management Program.

Visit will be paid at 100% when provided by a DakotaCare provider.

You will be responsible for additional cost related to routine tests or lab work and any charges
over "Usual and Customary and Reasonable" (UCR).
Note: At your office visit, please inform the nurse to code the visit as "asthma" related
(493.X) in order for the claim to be paid accordingly.
You are encouraged to visit the Asthma Management website at www.hcmti.com/asthma for more information and to enroll. Members may utilize this site to access online
monitoring tools that may be useful in managing their asthma.
