Weight Management


Since the mid-seventies, the prevalence of overweight and obesity has increased sharply for both adults and children. Studies show that among adults aged 20-74 years the prevalence of obesity increased from 15.0% to 32.9%.

As you know, being aware of your health status and personal health risks is the first step toward becoming healthier – and ensuring a better quality of life over the long term. The good news is that even minimal weight loss and exercise can improve or prevent complications associated with being overweight. By participating in the Weight Management Program, you can improve your health and quality of life. The program helps you to facilitate weight loss through diet, exercise and behavior modification. If desired, one of our health coaches will help you implement changes to increase your quality of life.

Members who have a BMI (Body Mass Index) of 35 or greater will be invited to participate in the Weight Management Program. This program provides counseling, educational materials, and other support to help participants improve their health. As an incentive for participation, the following expenses will be covered at 100% when provided by an in-network provider.

Member Benefits:
To begin utilizing the following benefits, you must enroll.
  • One (1) office visit per plan year (height, weight, blood pressure required) - paid at 100% when provided by an in-network provider. CPT Codes: 99201-99205, 99211-99215, 99241-99245, 99382-99404
  • One (1) HbA1c (glucose 3 month average) - paid at 100% when provided by an in-network provider. CPT Code: 83036
  • One (1) fasting blood sugar (fasting glucose) - paid at 100% when provided by an in-network provider. CPT Code: 82947
  • One (1) lipid profile (cholesterol, LDL, HDL, and triglycerides) - paid at 100% when provided by an in-network provider. CPT Codes: 80061 and/or 82465
  • Up to three (3) visits to a dietitian or nutritionist per plan year.
  • One (1) visit to your local health screening per plan year. For additional information regarding health screening events, for South Dakota Employee Health Plan members go to www.bopweb.com, for Associated School Board members contact your business manager.
  • This program is ongoing until BMI falls below 35 or member opts out.
Program Participation Process

Step 1
Member must see a physician for a physical evaluation. (May use the one 100% covered office visit per plan year at this appointment.)

  • Your physician should recommend an exercise program according to your fitness level to be preformed a minimum of three times a week for 30 minutes.
  • Once your physician has evaluated your current health status - enter your height, weight, and blood pressure on the Weight Management Website. It may also be submitted by email or USPS.
Step 2
Member begins and maintains exercise routine.
  • You can begin an exercise routine and log your workouts on the Weight Management Website as you go. It may also be submitted by email or USPS.
  • A minimum of 90 minutes of exercise per week. (Or physician's recommendations)
Step 3
Member submits behavior modification summary.
    What's a Behavior Modification Summary?
    - A weekly journal where members can express their achievements, goals, concerns, and frustrations.


  • A behavior modification summary may be entered on the Weight Management Website. It may also be submitted by email or USPS.
Step 4
Member submits food diary.
  • Submit a food diary every two (2) weeks that consists of four (4) days per week. This information can be entered on the Weight Management Website. It may also be submitted by email or USPS.
Step 5
Member sees a dietitian or nutritionist. (Recommended)
  • Up to three (3) visits to a dietitian or nutritionist per plan year are covered at 100%. Member must be enrolled in program for at least two (2) months before first visit.
